Instability - Specifically in the thumb saddle joint (CMC -I)
The thumb saddle joint is very mobile, this makes it possible to move the thumb in different ways. However, the joint can easily become unstable because it has a relatively weak ligament and joint capsule. Because of this, the thumb can often pop out of its bowl. It can become so unstable that you cannot even hold things properly anymore. The pain is below the thenar.
Causes
Thumb saddle instability is usually caused by congenital weakness of the ligaments, dislocation of the thumb in combination with a trauma. Research will show how unstable the thumb is and whether there is joint wear.
Treatment
Splinting the thumb is not a permanent solution, with prolonged use it will result in muscle weakness and more instability. However, they can be a good addition for when your thumb has been dislocated or for surgery. During surgery, a new ligament is made by taking a little piece of your tendon. This will be pulled through the tendon, in which a hole is drilled, and attached to a different tendon. This creates a new ligament that holds the saddle joint in place.
After treatment
Complications such as bleeding, tissue decay, narcosis problems, thrombosis, wound infection or pneumonia can occur during surgery. Fortunately, these complications are not common and can be treated well. After about three weeks of having a plaster cast after the surgery, your therapist will give you a detachable splint.
